  • Your name
  • List of medications you need a repeat prescription for
  • Pharmacy you want the prescription faxed to
  • If you wish to pick the prescription up at the clinic please advise us of this.

Under normal circumstances, repeat prescriptions will be provided on the script line (phone request) only if you have been reviewed in-person in the the clinic within the last 6 months.  If not you will be advised to book-in for a review before a prescription can be issued.  If booked-in for such a review of medications, please advise us if you may be running out of your regular medications before the fresh prescription can be issued at the review.  We may be able to issue you a prescription for a few days/weeks to bridge the gap until your review, where a prescription for 3 months can be issued.

Repeat prescriptions for certain medications may require even shorter time-periods before which an in-person review may be required.
